Q. What kind of insurance does 121 Care accept?
121 Care is an approved Medicare provider. We accept over 3,500 different national insurances. Please contact us directly to verify your benefits and eligibility for prosthetic services.
Q. How do I obtain a referral and schedule an appointment at 121 Care?
121 Care is required to have a Dispensing Order (referral) from your referring physician. You will need to contact your physician to schedule obtain a Dispensing Order. Once you have received the Dispensing Order from your physician, 121 Care can schedule you immediately for an evaluation appointment.
Q. Is your facility wheelchair accessible?
Our facility is ADA compliant, and easily accessible for wheelchair users. Our patient care facility is accessible via an elevator.
Q. Is there available parking in your building?
We currently do not have designated parking for our patients. There are blue zones within 1 block of the building. If you have a handicapped placard or plates, you are not subject to time limits or meter costs. However, you are required to abide by SFMTA rules, including street cleaning, tow zones, and restrictive meters of the following color: yellow, white, and red.
Q. Do you provide transportation to and from appointments?
121 Care does not directly provide transportation to and from appointments. We work with multiple third party ride services to provide free or low-cost options to our patients. We recommend contacting SF Paratransit or Redi-Wheels directly to setup an account. If you require additional assistance, please contact our Office Administrator.
Q. Can 121 Care put me in touch with another patient or support group?
Yes. 121 Care has a network of over 40 patient ambassadors. The majority of them are Certified Peer Visitors with the Amputee Coalition, and are trained to provide peer-to-peer support for others living with limb loss. If you would like to be connected with an ambassador, please let our Office Administrator know, and we will connect you with the appropriate candidate.
